Transitional Care in Cystic Fibrosis

In this webinar, Ms Wooldridge, a CF nurse specialist, focuses on the need for transition of adolescents and young adults with Cystic Fibrosis to move from child-focused care to adult-oriented healthcare. She explores why this transition is so important to meet the needs of an increasing adult population, in order to manage the added challenges that becoming an adult introduces.

Ms Wooldridge also focuses on how the transition pathway looked like pre-COVID between Birmingham Children’s Hospital and Heartlands Hospital or The Royal Stoke, and explains the successful set up of a virtual platform during pandemic along with its pros and cons.
